Kostyshyn Jaroslav Myroslavovych was the mobilization officer department 649 th Aviation of missile weapons and ammunition Air Forces of Ukraine, Colonel (posthumously).
He was born on June 19, 1980 in Ivano-Frankivsk. In 1998 he graduated the secondary school of the village Zahvizdya, Tysmenytsya district of Ivano-Frankivsk region.
Since 1998, he was in the Armed Forces of Ukraine. In 2002 he graduated from the Lviv Military Order of the Red Star Institute of Hetman Sahaidachny National University "Lviv Polytechnic" (now - National Army Academy named after hetman Sahaidachny), specialty "Automobiles and Automobile Industry."
Since 2002, he served in the 649th Aviation composition missile weapons and ammunition Air Force of Ukraine (military unit A3013 village Hruzevytsya Khmelnitsky region Khmelnitsky region).
On September 2014, Major Kostyshyn was voluntarily enlisted into the consolidated group "Wild Duck" Air Force of Ukraine, which was to serve in a combat zone in eastern Ukraine.
On September 25, 2014 he took part in anti-terrorist operation in eastern Ukraine.
In October 27, 2014, Major Kostyshyn died from shrapnel wounds during the shelling of the Russian-Ukrainian terrorist group of military positions near Avdeevka Donetsk region near the Donetsk airport.
On October 29, 2014 he was buried in the cemetery of the village Tomashivka Yarmolinetskiy district of Khmelnitsky region.
On October 16, 2015 in the village Zahvizdya Tysmenytsya area in front of the building of secondary school (street hills, 5a), where Jaroslav Kostyshyn studied, the memorial plaque was established.
He was awarded the Order "For courage" of III degree (27/11/2014, posthumously).
