Matlak Oleh Serhiyovych was the commander of the combat vehicle - the commander of the 2nd platoon of the 2nd Motorized Infantry Company of the 40th Separate Artillery Brigade of the Land Forces of the Armed Forces of Ukraine, a senior soldier.
He was born on 25 April, 1987 in the city Zhovti Vody, Dnipropetrovsk region. In 2003, he graduated the 9th form of the Zhovtovodsky Primary Education Complex No. 6 "Perspective", in 2005 - Higher vocational school №70 of the city Zhovti Vody, specializing in electro gas welder. He worked as an electric gas welder.
He served the regular military service in the Armed Forces of Ukraine.
In April 2015, he was mobilized to the Armed Forces of Ukraine. He served as the commander of a combat vehicle - the commander of the 2nd platoon division of the 2nd motorized company of the 40th separate artillery brigade of the Land Forces of the Armed Forces of Ukraine (military unit B3500, the city Pervomaysk, Mykolaiv region).
Since the summer of 2015, he participated in the anti-terrorist operation in eastern Ukraine.
On 26 August, 2015, the senior soldier Matlak died from a deadly wound as a result of a firing from the missile system of the volley fire "Grad" near the village Prokhorivka, Volnovakha district, Donetsk region, when a shell hit him in the cemetery. Along with him 4 more soldiers died.
On 4 September, 2015, he was buried in the city cemetery of Zhovti Vody.
He was awarded the Order of "For Courage" of the 3rd class (16.01.2016; posthumously).

On 1 June, 2016, in the city Zhovti Vody, on the facade of the building of the educational complex №6 "Perspective" (Yaroslav Mydriy Street , 11a), where Oleh Matlak studied, the memorial plaque was established.
