Polishchuk Yuriy was the chief sergeant, the division commander of the 54th separate reconnaissance battalion of Army of the Armed Forces of Ukraine, Ensign.
He was born on 3 August 3, 1982 in the village Tokariv, Novograd Volyn district,Zhytomyr region. He graduated the secondary school of the village Tokariv.
He was in the Armed Forces of Ukraine since the early 2000s. As of May 2014 he served as a chief sergeant - commander of the division of the 54th separate reconnaissance battalion of the Land Force of Ukraine (military unit A2076; Novograd Volyn, Zhytomyr region).
In 2007 he took part in the peacekeeping mission in Kosovo.
He lived in his native village Tokariv. He was captain of the football team of village Tokariv "Granit".
In May 2014 during a training exercise in the Zaporishya region he suffered a severe head injury. He was treated in the neurosurgery department of Zaporizhzhya Regional Hospital, where he died May 25, 2014, without leaving a coma.
On 28 May, 2014 he was buried in the cemetery of the village Tokariv, Novograd Volyn district of Zhytomyr region.
He was awarded the Order "For courage" III class (06/04/2015, posthumously).

On 28 May, 2015 in the village Tokariv on the building of secondary school (street Shevchenko, 17), where Polishchuk Yuriy studied, the memorial plaque was established. In the village of Tokariv there was held the tournament of mini-football devoted to his memory.
